- Full signature of the Candidate on White Paper in Black Ink Pen.
- HSCE or equivalent pass certificate and mark sheet issued by the Board.
- CHSE or equivalent pass certificate and mark sheet issued by the Council.
- Leaving Certificate of last institution attended.
- Domicile/ Residential / Nativity Certificate from competent authority.
- Caste certificate in respect of reserved category.
- PWD Certificate in case of persons with disability candidates.
- Service Certificate in case of Ex-servicemen/Serviceman.
- Green card certificate for Green Card Holders.
- Valid Aadhar Card
- NUID Card (Only M.Sc. Nursing & P. B. B.Sc. Nursing Admission Aspirants)
- Valid E-mail Id.
- Migration Certificate, Admit Card of Board & Council in case of outside Odisha candidates
- Physical Fitness Certificate / Medical Fitness Certificate.
- No Objection Certificate from the employer (only for P.B.B.Sc. & M.Sc. Nursing Course)
- GNM Pass Certificate, Mark Sheet & Valid Registration Certificate (only for P.B.S.Sc. Nursing Course)
- Certificate & Mark Sheet of GNM & P.B.B.Sc. (N) / B.Sc.(N) and valid Registration Certificate of P.B.B.Sc. (N) / B.Sc.(N) (only for M.Sc. Nursing Course)
- One Year Clinical / Teaching Experience Certificate from the date of registration to last date of application after completion of P.B.B.Sc. (N) / B.Sc. (N) course (only for M.Sc. Nursing Course)
